Forms & Permits

All student organization on-campus events require the following:

  • Facilities Use Permit or equivalent authorization to utilize a specific facility or space
  • Event Insurance Coverage (FAQs)
    • For low-risk events that are held on-campus, no Certificate of Insurance is necessary.
    •  For moderate- to high-risk events on campus, a Certificate of Insurance must be obtained.  Registered student organizations may obtain a Certificate of Insurance by going to theMarsh CampusConnexions website or by calling the Marsh Student Coverage Line at 866.838.9536 between 6am and 3pm, Monday through Friday.
